Accelerated examination

Under 37 CFR 1.102(c) it is possible to file a Petition to make
Special based on applicant's age if the applicant is 65 or older. And
there is no fee for such petition.

Evidence is required to prove the age. I understand that a copy of the
birth certificate, for example, or of a pass****t page or the like
would be acceptable.

I have not found a Form suitable for filing of such petition amongst
the forms provided on the USPTO site. Is there such a form, or will a
simple letter petition be sufficient?
